Benefits of VEI employment

We know that patient focused medicine begins with our employees. Therefore, we provide a competitive salary structure, along with a 401(k) retirement plan and a wide variety of benefits, including medical, dental, vision, short- and long-term disability, and life insurance.

Eligibility

  • 16 to 39 hours per week = Part-time benefits eligible
  • 40 hours per week = Full-time benefits eligible

FlexChoice benefits

  • Medical Insurance
    Comprehensive coverage available for you and your eligible dependents.
  • Dental Coverage
    Coverage for preventive, restorative and orthodontic care for you and your eligible dependents.
  • Vision Plan
    Coverage for vision care services such as routine eye exams, eyeglasses and contact lenses for you and your eligible dependents.
  • Flexible Spending Accounts (FSA)
    You set pre-tax dollars aside from your paycheck and reimburse yourself for qualified expenses (health care and dependent care), or pay from your FSA with Debit Flex Card.
  • Accidental Death & Dismemberment
    Term life insurance protection for you and your dependents for protection from accidents.
  • Long-Term Disability (LTD)
    Coverage up 50-70% of base monthly pay if you are unable to work.
  • Short-Term Disability (STD)
    Coverage of up to 60% of base monthly pay.
  • Hyatt Legal Plan
    By using in-network plan attorneys, plan provides legal consultations and services for guardianships, wills, consumer protection matters and many other legal services.
  • Life Insurance
    Basic Term, Additional Term and Dependent Term life insurance coverage.
  • Pre-Tax Payment of Premiums
    Pay premiums for medical, dental, vision, term life, accidental death and dismemberment and disability insurances, plus flexible spending accounts, on a pre-tax basis. Your taxable income is reduced, meaning lower taxes for you.

Retirement/Savings benefits

  • 401(k) Plan
    A Defined Contribution Plan comprised of three components: 401(k) deferral, match and profit sharing contribution. The plan helps save pre-tax dollars from your salary along with additional retirement contributions.

Work/Life benefits

  • Assisted Fertility Services
    Services to benefit eligible employees and their spouses. The discount will apply to any services offered by Community's Assisted Fertility Services program and its physicians and lab.
  • Bariatric Surgery Discount
    Services to benefit eligible employees and their spouses. The discount will apply to the total on the final billed charges when surgery is performed by one of the physicians who specialize in this procedure for Community. More about bariatric services

    Discounts on child care services provided at reduced child care costs at LaPetite Academy and other participating child care centers.
  • Child Care/Elder Care Resource and Referral
    Telephone information and referral for child care and elder care services.
  • Child Sick Care
    Sick child care services provided in the Pediatric Departments of Community Hospitals.
  • Tuition Reimbursement
    Reimbursement may be available for employees to receive partial or total reimbursement for approved tuition fees up to program maximum for each location.
  • Employee Assistance Program (EAP)

    Answers to your financial questions from a MetLife Securities, Inc. representative.
  • HealthQuest
    Health Promotion programs offered on a voluntary basis that focus on wellness and healthy lifestyle choices and behaviors. You may qualify for Wellness Discounts on your medical insurance premiums based on screening results.
  • ID TheftSmart
    A five-way defense plan that offers the services of licensed investigators who are trained to recognize the intricacies of identity theft and resolve the problems that develop as a result.
  • Indiana Family College Savings Plan
    Save for your children's post-high school education with this state program. Earnings aren't taxed until account is used, typically when a student's tax rates are lower than his or her parents.
  • Life Insurance-Whole Life for You and Your Dependents
    Interest-sensitive, whole life insurance through Boston Mutual with advantages of cash accumulation at current interest rates. Available to employee, spouses, dependent children and grandchildren; subject to plan guidelines. Coverage is portable.
  • Long-Term Care Insurance
    Provides coverage for you and/or your family for long-term care in nursing facility or home health care.
  • Paid Time Off (PTO)
    You accrue time off each pay period that you work. You can "bank" that time and use it for approved, scheduled time off for purposes such as vacation, illness, and personal or family needs.
  • Pet Insurance
    Health care coverage for your pets, including dogs, cats, birds and many other animals.
  • Property and Casualty Insurance
    Individual policies for auto, homeowner's and other property/casualty coverage. Premiums are paid through payroll deduction.
  • U.S. Savings Bonds
    Use savings bonds as part of your regular savings plan. An optional benefit is the Bond Continuation Insurance Program that continues to purchase bonds for 10 years in the event of an employee's death.

Other benefits

  • Business Travel Insurance
    Accidental death and dismemberment insurance providing coverage while you are traveling on company business (subject to exclusions and limitations stated in the policy).
  • Liability Insurance
    Insurance to protect you in case you are sued as an individual as a result of acts you perform within the scope of your employment (not including intentional tortious or criminal acts).
  • Workers' Compensation
    Pay for all approved medical expenses due to an illness or injury incurred in the course of your employment, plus pay for your lost work time in accordance with state law.
