Michigan new hire information - Where to go and what to bring

Health assessment

A pre-employment health assessment must be completed before your start date. Health Assessments can be conducted at one of the four locations below. When you call to schedule an appointment, please let them know you are a new VEI employee.

Please bring with you:

  • Driver's license
  • Completed health history form. Completing the health history prior to your appointment will expedite your appointment.
  • Copy of PPD done within the last year (if available – will ONLY be used as 2nd step. 1st step TB test MUST be placed by Employee Health)

  • Authorization to release information 

Your paperwork appointment will take place at the facility at which you have been hired to work. You will need to call to schedule an appointment prior to your start date.

Please bring with you:

  • Driver’s license**
  • Social Security card**
  • Bank routing number and account number for direct deposit. Please check with your bank for the routing/transit number and account number to use. You should never use a deposit ticket for the information. The routing number listed on the deposit ticket is often an internal banking number and will not work for direct deposit. Incorrect information delays the implementation of your direct deposit and could even cause a delay in receiving your paycheck.
  • License/certification for the position (if applicable)
  • Diploma or official transcripts as instructed by your recruiter

You must bring your diploma or official transcripts in addition to your licensure or certification for the position.

** Please note: A social security card or receipt of application is mandatory for employment. The last name on the driver’s license and Social Security card must match. If not, you will not be able to start until you provide HR with an updated driver’s license or Social Security card/request receipt (two pages). If you bring this receipt, HR also needs to make a copy of your birth certificate.

If you need to obtain a replacement Social Security card, name change, or if you do not have a Social Security number, you may visit the Social Security Administration online for more information.

Post-new hire paperwork

  • Contact your department manager.
  • Your supervisor is available to assist you with any questions about your schedule or additional requirements.
